Firstly, Dickens' choice of adjectives which describe Scrooge's life are decidedly depressing. The repetition of the word "Melancholy" in the first sentence

Its metaphorical because it is trying to portray that Scrooge is literally as tight as the hand Scrooge, upset by hearing Belle's description of the man that he has become, demands that the ghost remove him from the house.
